I wonder just how practical, controllable, and safe, ballbusting is using a blood pressure measuring machine

I’ve found very little information, mainly this clip,

The machines should have an off button and if they do not you can easily pull the air pressure cord out of the machine. The problem is that the arm cuffs are too big.
These are smaller cuffs like this but I have never tried them. Search for Vacuum Pressure Cuff Set.

This is probably safer because it does not involve a machine. If you really have your heart set on using a machine then you will need a blood pressure machine meant for wrists or small arms. Unfortunately the machine will stop and deflate earlier than you would probably like. The manual pumping device will not.

Although I found a lot of people theorizing about it, I could not find a single first-hand account of the experience. I suspect that a cuff or restraint of some kind would be needed to prevent the testicles from going back toward the body.

You are right: one can see in the video that she has set a small rope around his balls and that she kind of pulls them inside the blood pressure cuff, probably to maintain them in place.
